Welcome to Nu Image Hair Rehab Rx (“the Clinic,” “we,” “our,” or “us”). These Terms & Conditions (“Terms”) govern your access to and use of our medical services, consultations, procedures, website, and related offerings (“Services”). By receiving treatment, booking an appointment, or interacting with the Clinic, you agree to these Terms.

If you do not agree with any portion of these Terms, you should not use our Services.

1. Not a Guarantee of Results:
   Outcomes of hair restoration or alopecia treatments vary by individual. We do not guarantee specific results, timelines, levels of improvement, or permanence.
    
2. Individualized Treatment:
   Treatment plans, supplements and procedures are tailored to each patient’s medical condition. Your compliance with pre- and post-treatment instructions directly impacts outcomes.
    
3. Medical Advice:
   Information provided by the Clinic (verbally, in writing, or online) is for educational purposes and does not replace personalized medical advice.
    
4. Suitability of Treatment:
   The Clinic reserves the right to decline treatment if a patient is medically unsuitable or if treatment poses unreasonable risk.
    

1. Booking Requirements:
   Appointments must be booked in advance.  A deposit is required for all services if full cost is not paid.
    
2. Cancellations:
    
   • Cancellations or rescheduling must be made at least 48 hours prior to the appointment.
      
   • Late cancellations or no-shows may result in agreement termination.
      

1. Late Arrivals:
   Patients arriving more than 30 minutes late will incur a $50 fee at the time of service.
    

1. Payment Policy:
   Payment is due at the time services are rendered unless arranged otherwise in writing.
    
2. Deposits:

              A non-refundable deposit is required to secure your appointment.
 

1. Refunds:
    

              There are no refunds after a contract has been signed.  Consultation fees are

              non-refundable.                
 

1. Insurance:
   Hair restoration treatments  are generally considered cosmetic and are not covered by most insurance plans unless otherwise specified.
    

By using our Services, you agree to:

• Provide accurate and complete medical and personal information.
   
• Disclose medications, medical history and most recent blood test from your physician, allergies, prior procedures, and conditions relevant to treatment.
   
• Follow all pre-procedure and post-procedure care instructions.
   
• Inform the Clinic immediately of adverse reactions or complications.
   
• Treat staff respectfully and refrain from harassment, abuse, or disruptive behavior.
   

Before any treatment, you will be required to sign a Medical Consent Form that includes:

• Explanation of the treatments
  Alternatives to treatment
  Post-treatment care responsibilities
  You have the right to refuse or discontinue treatment at any time.

All medical and cosmetic treatments involve some degree of risk. Potential risks may include, but are not limited to:

• Swelling, bruising, redness, or discomfort 

• Allergic reactions
   
• Need for additional treatments
   
• Your provider will explain specific risks based on your treatment.

We comply with applicable patient privacy and data protection laws (HIPAA)

• Your medical information is confidential and will not be shared without your consent except as required by law.
   
• By using our Services, you consent to the collection and storage of personal data necessary for treatment and clinic operations.
   

The Clinic may take clinical photographs or videos before, during, or after treatment to document your progress.

• These images are part of your medical record.
   
• The Clinic will not use identifiable images for marketing without your written consent.
   

By using our website, you agree not to:

To the fullest extent permitted by law:

The Clinic may refuse or discontinue treatment if:

• The patient is non-compliant or abusive
   
• Providing treatment would be unsafe
   
• Payment terms are not met
   

You may terminate your relationship with the Clinic at any time. However, all monies are still due as per the contractual agreement.

The Clinic may update these Terms at any time. Changes take effect upon posting and apply to all future services. You are responsible for reviewing updates periodically.

These Terms are governed by the laws of  New Jersey without regard to conflict-of-law principles. Any disputes shall be resolved through [mediation/arbitration/court jurisdiction].
